When coming to your appointment please bring:

• Doctor's referral if applicable (please note a referral is not necessary to see a physiotherapist)
• Any current related xrays, scans or reports
• Comfortable, loose clothing (to allow exposure of the area to be treated)
• Private Health Fund card for on-the-spot rebates (Hicaps).
• Workcover Certificate and insurance company information for Workers Compensation claims
• EPC referral, Medicare card and Debit Card for rebate
• DVA Gold Card and referral if applicable
• Out of respect for others, please turn off mobile phones during appointments
• Please note that treatment charges are payable at the time of service
